There are two "click of death" sounds:
a) Insert disk, two clicks and then silence (no disk spinning)
Cause: Can not read "Z" tracks [this holds bad sector info]
Solution: Waranty claim if possible - the drive is DEAD!

b) While accesing the disk, two clicks are heard, disk keeps spinning
Cause: "Drive parks the heads and initalises the disk sequencer
              during error recovery routines."
Solution: Perform a "full certification" format on the disk

The other two OOPS appear to be problems somewhere in the mid-level
SCSI driver.
