No guarantees that it will solve all the problems, but you should turn off
the "probe all luns" config option. It looks like the drive got confused
when the kernel tried to probe lun 1 - and then everything fell apart.
Hint: you might also want to enable some of the debugging options in
the driver - they can help to track down where things are failing.
Reset processing is one of the known areas where that driver simply does
not work.
